This is a horseshoe modified to prevent injury in horses which brush or interfere with the inner (medial) heel region of a hoof. 

The inner heel is fitted tight, i.e. there is no protrusion of the outer edge of the shoe over the bearing edge, and the ground surface is safed (rounded) to a greater degree than usual.
